    Former Health Minister Urges £16 Billion NHS Modernisation Plan

    THE FORMER Health Minister, Lord Hutton, has called for an NHS Rapid Upgrade Programme to unlock up to £16 billion of private investment, urging the Government to modernise NHS hospitals through existing PFI contracts rather than waiting years for new-build projects. 

    Health Minister’s Call to Accelerate NHS Modernisation 

    Lord Hutton told delegates that Britain had become trapped debating the past rather than delivering the infrastructure patients need. NHS Trusts continue to face deteriorating buildings and rising costs, with the maintenance backlog standing at around £16 billion. The Association of Infrastructure Investors in Public Private Partnerships (AIIP), which backs the proposal, has urged the Government to use existing PFI contracts as a foundation for faster modernisation. 

    Proposed Extension of Existing PFI Contracts 

    AIIP proposed extending contracts across 151 NHS PFI projects due to expire this Parliament, arguing structured extensions could let investors refinance schemes and reinvest capital into hospital upgrades. Lord Hutton contrasted the original programme, under which around 90 hospitals were delivered in under a decade, with current replacement projects, some of which had spent over a decade working towards construction. AIIP wrote to Health Secretary James Murray MP in June, urging reforms to strengthen Trust-investor relationships. 

    Modelling Points to £16 Billion Investment Potential 

    AIIP’s analysis indicated that extending the 151 eligible contracts could unlock £15.4 billion, close to the NHS’s entire maintenance backlog, with individual Trusts able to receive up to £180 million in upfront capital through refinancing senior debt and unwinding legacy swaps from the 2012 deals. In one illustrative scenario, an authority received an upfront payment of £132 million, ring-fenced for asset repurposing or used to reduce annual charges. Budgetary treatment under HM Treasury’s FReM 2025-26 avoided the capital “spike” typically required for post-expiry replacement, though nominal charges over the asset’s life increased under an extension. 

    Implications for Future NHS Infrastructure Investment 

    The proposal has emerged as NHS leaders continue to grapple with ageing estates and constrained finances, with AIIP arguing that extending PFI contracts could shift delivery risk to the private sector while accelerating Net Zero upgrades. Not everyone has shared this optimism. Siva Anandaciva of The King’s Fund has said the backlog exceeds the current capital budget, warning that delays to the New Hospital Programme continue to leave patients in limbo.
