Understanding the difference between exercise and overtraining
Exercise is described as medicine for the heart, and for good reason. Regular physical activity can lower blood pressure, improve fitness and reduce the risk of heart disease and stroke. But there is a point where the question changes from “How much exercise is good?” to “Is the body getting enough time to recover?”That distinction matters, especially as intense workouts, marathon training, long-distance cycling, high-intensity gym sessions and fitness challenges become increasingly popular. The pressure to run farther, lift heavier or train every day can make exhaustion look like progress.The heart normally responds to exercise by beating faster and pumping more blood. That is a healthy adaptation. The concern is not that exercise itself is harmful. It is what can happen when very demanding exercise becomes a repeated physiological stress, particularly when recovery, hydration, nutrition and sleep do not keep pace.“Exercise increases the workload on the heart, but this is a healthy, normal physiologic response. It’s not the length of an exercise, but what happens to the cardiovascular system when it is subjected to extreme physiological stress on a regular basis,” said Dr Devika G, Cardiologist at HOSMAT Hospital.So, where does healthy training end and excessive training begin?

When a hard workout stops being a healthy challenge
A hard workout is not automatically an unsafe workout. In fact, the heart is supposed to work harder during exercise. Over time, regular training can improve how efficiently the cardiovascular system delivers oxygen to the muscles.The problem arises when demanding sessions keep piling up without enough recovery.Dr Ashutosh Kumar, Head – Cardiac Electrophysiology and Arrhythmia Unit, Arete Hospitals, explained that the concern grows when training intensity or volume remains high while the body gets too little time to recover.“Exercise is one of the best things a person can do for cardiovascular health, but more exercise is not always better. The concern arises when the intensity or volume of training is consistently high and the body is not given enough time to recover,” he said.This can happen in several ways. Someone may suddenly move from occasional running to daily long-distance runs. Another person may combine heavy strength training with intense cardio every day. Some people keep increasing their workload because a lighter session feels like they are “not doing enough”.

Why extreme exercise can affect the heart
The heart is remarkably adaptable. During exercise, it increases its rate and pumping capacity to meet the muscles’ higher demand for oxygen. Regular training can also produce changes in heart size and function that are normal adaptations in trained people.But extreme and prolonged endurance training may create a different situation in some individuals. This does not mean that running, cycling or other endurance exercise is dangerous. Nor does it mean that recreational athletes should fear exercise.It means the cardiovascular system is not a machine with unlimited tolerance. The benefits of physical activity are well established, while the effects of very high training loads can depend on the person, the training pattern and underlying health.Dr Kumar put this distinction clearly, “The heart is designed to adapt to exercise, and in trained individuals, some changes in heart size and function are a normal response to regular training. However, very high volumes of endurance exercise over long periods may have consequences in some individuals, particularly when there is an underlying or previously undiagnosed cardiac condition,” he said.

Heat, dehydration and stimulants can change the equation
The workout itself is only one part of the cardiovascular load. A long or very strenuous session, especially in hot and humid weather, can lead to significant fluid loss. Dehydration can alter the body’s electrolyte balance, including sodium and potassium levels. These minerals play important roles in normal electrical activity in the heart.“When prolonged or very strenuous exercise is combined with hot, humid conditions, dehydration and electrolyte changes (sodium and potassium) can affect electrical activity of the heart and increase the risk of rhythm problems,” Dr Devika said.This becomes particularly relevant for people who train outdoors in extreme weather or undertake long endurance sessions without an appropriate hydration strategy. Then comes another increasingly common issue: stimulants.Energy drinks, high-caffeine pre-workout products and some performance-enhancing substances can add another layer of stress. Caffeine can raise heart rate and blood pressure in some people, particularly at higher doses or in sensitive individuals.Dr Devika warned that these products can become particularly concerning when combined with fasting, dehydration or inadequate nutrition.“Some stimulants and high doses of caffeine can raise blood pressure and heart rate and add to the cardiovascular strain of an already challenging workout. They can be very dangerous when combined with fasting, dehydration or poor nutrition,” she said.

Fitness does not make someone immune to heart problems
One of the most dangerous assumptions in fitness culture is that being lean, strong or capable of running long distances automatically means the heart is healthy.It does not.Some heart conditions can remain silent until a person places the cardiovascular system under considerable stress. A genetic tendency to an abnormal rhythm, an underlying structural problem or another previously undetected condition may not produce obvious symptoms during everyday life.“Physical fitness does not provide perfect protection against cardiovascular events. Even a very fit person can have a genetic predisposition to an irregular heartbeat or other heart weakness,” Dr Devika said.This is why copying the training routine of an elite athlete can be a poor idea for someone with a completely different age, medical history and fitness base.Dr Kumar said exercise intensity should not be determined by comparisons with athletes or fitness influencers. It should reflect a person’s age, baseline fitness, medical history and training goals.The body also gives clues when something has changed.“If a person who was previously able to complete a particular workout comfortably is suddenly struggling, that change should be paid attention to rather than trying to push through it,” Dr Kumar said.He also issued a particularly important warning about fainting and giddiness.“Any episode of giddiness or fainting should be taken very seriously as may be penultimate symptom to cardiac arrest. For recreational athletes and serious endurance enthusiasts alike, recognising when the body is asking for rest can be an important part of protecting long-term heart health,” he said.A red flag does not mean a person has a serious heart condition. It means the symptom deserves proper assessment rather than being ignored.

The goal should be a stronger heart, not a constantly exhausted body
Training should therefore progress gradually rather than jumping sharply from one workload to another.Sleep matters. So does adequate food. Hydration matters even more during long sessions and hot weather. Stimulants should not be used casually to force the body through fatigue.And sometimes the smartest training decision is to stop.That can feel uncomfortable in a culture that celebrates pushing past limits. But knowing when not to push is also a form of fitness.
