Are cholestrol levels and metabolic disorders related?AI Narration
    Listen00:00

You get your cholesterol tested, and the numbers look fine. But does that mean your metabolic health is in the clear, too? Not necessarily. Cholesterol levels may offer important clues about how the body processes sugar and fat, raising questions about their connection with conditions such as diabetes and metabolic syndrome

Can cholesterol levels signal a metabolic problem before diabetes is diagnosed? And what if your cholesterol report comes back normal?

This is also what prompted an internet user to ask on Quora:Is there a connection between cholesterol levels and metabolic disorders?

We reached out to Dr Aarti Ullal, physician and diabetologist at Gleneagles Hospital, Parel, and Dr Rajiv Kovil, diabetes and obesity specialist at Zandra Healthcare, to understand the connection in detail

How are cholesterol levels linked to metabolic disorders?

“Cholesterol levels are closely linked to metabolic health,” Dr Ullal tells indianexpress.com

When the body becomes resistant to insulin, the hormone that helps regulate blood sugar, it can also affect how it processes fats. This may lead to higher triglycerides, lower levels of HDL (good) cholesterol and changes in LDL (bad) cholesterol

“Cholesterol is not simply a problem of eating too much fatty food. Very often, it reflects how efficiently the body handles energy, sugar and fat,” Dr Kovil further explains

Conditions such as obesity, type 2 diabetes and metabolic syndrome can disrupt this balance. Excess fat around the abdomen and liver can also affect how the body produces and processes cholesterol and triglycerides

“These changes increase the risk of heart attacks, strokes and fatty liver disease,” Dr Kovil notes while explaining that cholesterol abnormalities may develop years before diabetes is diagnosed

Can abnormal cholesterol levels be an early sign of a metabolic problem?

Normal cholesterol does not automatically mean good metabolic health.

Yes, certain cholesterol patterns can indeed offer an early warning. High triglycerides combined with low HDL cholesterol may indicate insulin resistance, sometimes even before blood sugar levels become significantly elevated

“Abnormal cholesterol patterns can be an early warning sign of metabolic dysfunction, sometimes appearing long before blood sugar becomes abnormal,” Dr Kovil confirms

High triglycerides combined with low HDL cholesterol are particularly suggestive of insulin resistance,” the diabetologist further adds

However, not every cholesterol abnormality indicates insulin resistance. High LDL cholesterol can also result from genetics, thyroid disorders or other medical conditions. An abnormal cholesterol report may therefore warrant further evaluation for conditions such as prediabetes, abdominal obesity, fatty liver disease and metabolic syndrome

How does insulin resistance affect good and bad cholesterol?

Insulin resistance can affect all three major components of a lipid profile: triglycerides, HDL and LDL

The liver may release more triglyceride-rich particles into the bloodstream, raising triglyceride levels. HDL cholesterol may fall, while LDL particles may become smaller and denser, increasing their likelihood of contributing to plaque buildup in the arteries

“Interestingly, LDL cholesterol may remain within the normal laboratory range despite these harmful changes,” Dr Kovil notes

This helps explain why people with diabetes can face an increased risk of heart disease even when their cholesterol numbers do not appear particularly high. The problem involves not just cholesterol concentration but also how the body processes fats

Can you have normal cholesterol and still have a metabolic disorder?

Yes. A normal cholesterol report doesn’t automatically mean your metabolic health is good

“A person can have normal total cholesterol and still suffer from insulin resistance, prediabetes, fatty liver disease or even established diabetes,” Dr Kovil confirms

Some people may also have excess abdominal or liver fat despite appearing slim and having a normal lipid report

Dr Ullal further emphasises that cholesterol levels alone cannot provide a complete picture of metabolic health. Blood sugar, blood pressure, waist circumference and other health indicators must also be assessed

Dr Kovil adds that conventional cholesterol tests do not always reveal the number of potentially harmful cholesterol-carrying particles circulating in the blood. Therefore, a normal report should not be the only measure of metabolic health

Which tests can help assess cholesterol and metabolic health?

Screenshot of the query posted on Quora

A lipid profile measures total cholesterol, LDL cholesterol, HDL cholesterol and triglycerides. Fasting blood glucose and HbA1c help assess blood sugar levels, while blood pressure and waist circumference provide additional information about metabolic risk

Depending on a person’s health and risk factors, doctors may recommend further tests to investigate possible underlying problems

Regular health check-ups can help identify abnormalities early and guide timely treatment. If your cholesterol levels are outside the recommended range, ask your doctor whether they should also assess your blood sugar and other metabolic risk factors
