Most people look at their fasting blood sugar, post-meal blood sugar, and HbA1c levels, and if all three are within the recommended range, they believe there is nothing to worry about.But the body is often far more complicated than a few numbers on a laboratory report.A growing understanding of metabolic health suggests that blood sugar can remain perfectly normal even while the body is already heading toward metabolic dysfunction. It may happen years before diabetes is diagnosed. The body can hide problems surprisingly well
Blood sugar does not rise overnight.In many people, the pancreas spends years working overtime. It releases larger and larger amounts of insulin to keep glucose levels within the normal range. On paper, everything looks fine. In reality, the body is already struggling.This is known as insulin resistance.Insulin is the hormone that helps move glucose from the bloodstream into the body’s cells, where it is used for energy. When cells become less responsive to insulin, the pancreas compensates by producing more of it. This extra effort can maintain normal glucose levels for years.

Why insulin resistance deserves more attention
Insulin resistance is not only about diabetes. Research has shown that it affects several organs at the same time and may contribute to a range of metabolic disorders.According to Dr Sudhir Kumar, an elevated HOMA-IR, a commonly used estimate of insulin resistance, can serve as an early warning sign for conditions such as:Type 2 diabetesFatty liver diseaseCardiovascular disease, including heart attacks and strokesThis explains why someone may appear healthy during a routine blood sugar screening but still carry a higher long-term risk for chronic disease.

What are fasting insulin and HOMA-IR, and should everyone get them?
Dr Kumar points out that people interested in detecting early metabolic dysfunction may discuss two additional measures with their doctor:Fasting insulin: This measures how much insulin the pancreas produces after fasting. High fasting insulin may suggest that the body is compensating for insulin resistance.HOMA-IR (Homeostatic Model Assessment of Insulin Resistance): This is not a separate blood test. It is a calculated value based on fasting blood glucose and fasting insulin levels. Several validated medical calculators are available for healthcare professionals and researchers.However, it is important to understand that HOMA-IR is mainly a research and clinical assessment tool and is not recommended as a routine screening test for everyone. There is no universally accepted cut-off value because results vary depending on age, ethnicity, body weight, and laboratory methods. Doctors interpret it alongside a person’s medical history, physical examination, and other investigations.If someone has obesity, a strong family history of diabetes, fatty liver disease, polycystic ovary syndrome (PCOS), or other metabolic risk factors, a healthcare professional may decide whether these tests would provide useful additional information.

Perhaps the most encouraging part of insulin resistance is that it often responds well to lifestyle changes, especially when detected early. Before blood sugar becomes abnormal, the body may still have a chance to recover
