    REM sleep is not rest, but work. The brain consolidates memories, processes emotional experiences and stabilizes psychological resilience. Those who fail to reach it or leave it too early carry a burden the next morning that sleep should have lifted. Researchers at the University of Texas at Austin have developed a skin-breathable patch that uses focused ultrasonic waves to bring people into this sleep phase 43 minutes earlier and keep them there 16 minutes longer. No medication, no surgery, no bodily intervention.

    How NEUSLeeP Changes Sleep

    The device is called NEUSLeeP and adheres to the scalp like a patch. It combines two functions: focused low-frequency ultrasound penetrates the skull and reaches deep brain regions involved in controlling REM sleep. At the same time, integrated electrodes measure brain activity in real time and dynamically adjust stimulation to the current sleep pattern. This approach is novel. Previous non-invasive methods to influence deep brain structures were either too imprecise or too weak to selectively reach sleep-controlling regions.

    For the first time, we were able to non-invasively address deep brain regions involved in REM sleep while simultaneously monitoring brain activity, explained Kai Wing Tang, the doctoral student who led the research. His advisor, Professor Huiliang Wang from the Department of Biomedical Engineering at the Cockrell School of Engineering, is primarily responsible for device development. Also involved were Gregory Fonzo, psychiatry professor at UT’s Dell Medical School, and sleep medicine specialist Vincent Mysliwiec from UT Health San Antonio.

    What the Study of 28 Participants Showed

    Researchers tested NEUSLeeP on 28 people, including healthy sleepers and those with documented sleep problems. On average, participants reached REM sleep 43 minutes earlier than without the device and spent around 16 minutes longer in this phase. Brain scans additionally showed changes in emotional circuits linked to fear and stress processing

    These changes are particularly relevant for the planned applications. The team sees NEUSLeeP long-term as a potential therapeutic option for post-traumatic stress disorder (PTSD), depression and chronic insomnia. Results were published in Nature Communications (DOI: 10.1038/s41467-026-73787-6). A patent application has been filed; commercialization is proceeding through UT’s Discovery to Impact program.

    In Comparison: What Sleep Medicine Offers Today

    Approximately 5.7 percent of German adults meet the criteria for a clinical insomnia diagnosis according to the nationwide DEGS1 health study by the Robert Koch Institute. The established first-line therapy is cognitive behavioral therapy for insomnia (CBT-I), which combines sleep restriction, stimulus control and cognitive restructuring and leads to sustained improvement in the majority of affected individuals. Its drawback: several weeks of intensive support by trained personnel are required and availability is unevenly distributed regionally.

    Sleep medication, primarily benzodiazepines and Z-drugs, provide short-term symptom relief but create dependency and distort natural sleep architecture. Many sleep medications actively suppress REM sleep rather than promoting it. Non-pharmacological brain stimulation such as transcranial magnetic stimulation reaches the cerebral cortex but not deeper sleep-control regions. Deep brain stimulation via implanted electrodes is invasive and remains limited to conditions like Parkinson’s. NEUSLeeP closes a gap that no therapeutic option has filled so far: a non-invasive intervention that selectively increases REM proportion of sleep rather than suppressing it.

    Three Questions Larger Studies Must Answer

    A study with 28 participants is a proof of concept, not clinical evidence of efficacy. Three open questions determine whether NEUSLeeP actually enters therapy. First, does the effect persist over many nights? REM sleep systems can adapt to external stimulation; the current study measures only single nights. Whether the 43-minute shift remains measurable after two weeks of daily use must be shown by a long-term study. Second, does the device work equally well in severe chronic insomnia as in mild sleep problems? The 28 participants formed a heterogeneous group; a clinical trial would need to isolate diagnosis-based subgroups. Third, are the changes in emotional circuits clinically relevant for PTSD patients or people with depression? These are the patient groups the team sees the greatest benefit for. Without corresponding controlled trials, this connection remains promising but unproven. Researchers have announced larger follow-up studies. When NEUSLeeP enters clinical approval remains unclear.
